Medical Records & Administrative Inquiries

CleanSlate Centers understands that other providers, external agencies and patients may need access to medical records, credentialing and billing information. Please review the information below to understand our process for submitting and following up on requests, and contacting CleanSlate Center’s departments. Our goal is to provide prompt, accurate responses to enhance your experience with us. Please know that we cannot process most requests related to medical records via our main phone line.

MEDICAL RECORDS

Where do I send a medical record request to?

You can fax medical record requests to the number 866-576-8050 or e-mail us medicalrecords@cleanslatecenters.com

How do I follow up on a previous medical record request that was already sent?

If the medical record was faxed to 866-576-8050 or emailed to medicalrecords@cleanslatecenters.com, please allow us at least 2 full weeks to process this request before calling or contacting us. If it has been 2 full weeks, please contact medicalrecords@cleanslatecenters.com with the original request marking it as a follow-up request. We do not handle these types of requests or follow-ups over the phone.

I am a patient, physician or other external agency that has requested a medical record. How long will this request take? I have an urgent request.

If the request was placed with Call Center staff, the Center staff, faxed to 866-576-8050 or was emailed to compliance@cleanslatecenters.com, please allow us minimally 14 business days to process your request, however, if this is an emergency or urgent matter, please contact compliance@cleanslatecenters.com and include the following in the subject line: Following up Medical Record Request: Urgent

CREDENTIALING

How do I verify if a provider or physician is still active and the location(s) from which they provide services.

You can email us at hr@cleanslatecenters.com. We do not handle these types of requests over the phone.

I am a third-party vendor. How do I reach someone at CleanSlate related to the credentialing and enrollment process?

Please email credentialing@cleanslatecenters.com and give us 48 hours to respond.
