Patient Referral Fax
Fax the completed referral form with the patient’s information to (833) 296-0864. Once we receive the form, we will schedule the appointment and fax back the confirmed time.
Send the completed referral form with the patient’s details to patientsupporthelp@cleanslatecenters.com. We’ll email you the appointment time once it has been scheduled.
