We view addiction as a chronic disease and believe that long-term remission is achievable through evidence-based medical management, individualized to suit each patient’s unique needs We believe in integrated care delivery. Integrated Care

We view addiction as a chronic disease and believe that long-term remission is achievable through evidence-based medical management, individualized to suit each patient’s unique needs, with coordination and monitoring of behavioral therapy. At CleanSlate, we treat our patients with dignity, compassion and respect in a physically and emotionally safe environment. We provide care that is both necessary and medically appropriate.

Each patient will be evaluated and an individualized treatment program to meet their specific needs will be developed incorporating aspects of the four pillars of our care program, medical, counseling, community, and recovery.

Medical

The treatment of substance use disorder requires individualized plans addressing the specific needs of each patient. Our medical team of addiction specialists including physicians, nurse practitioners and physician assistants, provide evidence-based treatment bringing our patients state-of-the-art treatment keeping pace with innovations in the field. Counseling

A holistic approach requires identifying and addressing mental health conditions that exist along with substance use disorders. We are proud to offer a team of licensed counselors who are dedicated to doing just that. Our counseling services help patients to overcome challenges they are experiencing that affect their recovery and all aspects of their lives. Assisting patients to identify and achieve their goals is the primary focus of our counselors. CleanSlate offers individual, group and tele-counseling services. Our counselors have expertise in substance use disorder as well as associated mental health conditions such as depression, anxiety, grief, bipolar disorder, PTSD and more. Our counselors have a wide variety of backgrounds and training and use evidence-based treatment approaches to address patients’ needs. We are excited to offer this service so you can take that next step toward becoming the person you want to be.

Community

As part of CleanSlate’s holistic approach to healing from addiction, we offer Care Coordination services at most of our centers. We understand that addiction impacts many aspects of the lives of our patients. Care Coordinators or Targeted Case Managers will work with you to remove any barriers to treatment, help you to develop a plan of care and connect you with the proper resources, services and support to ensure your success. Examples of the services provided include assistance with transportation, referrals to community mental health providers and primary care providers, help with resume building and job searches, referrals of assistance with housing, utility payments and food when needed.

Recovery

Certified Recovery Specialists (CRS) walk alongside patients throughout their self-directed recovery journey. A CRS draws from their own lived experience with addiction and recovery to provide empathy, encouragement, and support in our centers and in the community.

Services provided include emotional support through in-person and virtual meetings, helping patients connect with resources, services and community supports, accompanying patients to meetings and appointments, working with patients throughout their journey to achieve short- and long-term goals.
