CleanSlate recognizes how valuable and important each of you, our Warriors for Hope are. You all live our mission statement daily in our fight to save patient lives and to help our patients live the lives they want and deserve. While giving care is honorable and rewarding it can also be difficult and can lead to stress and fatigue. And of course, all team members may face difficulties inside or outside of work at any time in their lives.
CleanSlate has heard your concerns and has made an important step to be of more assistance to you and to your family members. CleanSlate also wants to assist you in living the lives you all want and deserve and to that end, we are pleased to introduce a more robust and comprehensive Employee Assistance Program (EAP). Our hope is that this will provide each of you with some additional tools to use for a variety of items.
What types of things can the employee assistance program help with?

Who is eligible to use the EAP:
You are automatically able to utilize the EAP upon hire. Your spouse or domestic partner and any dependents under 26 may utilize the EAP. Dependents may use the EAP even if they are away at college
All eligible members have access to:
3 face to face-to-sessions with a licensed counselor per member, per issue. The assistance comes from a master?s level EAP professional and can be accessed 24/7
30-minute face to face or telephonic legal consult, this is 30 minutes per issue. You will receive a 25% discount with the assigned attorney should you wish to proceed with their services
