Know someone who would make an outstanding Warrior for Hope? Refer them to CleanSlate and earn a cash bonus!

Last year, CleanSlate paid out nearly $27,000 in referral bonuses to fellow employees that helped us fill a position.

Referral-Bonus-Program-1200x662CleanSlate recognizes one of our best sources for employee candidates is our current employees. If a referred candidate is hired and continues to be employed after the 90 day introductory period, the referring employee will receive a $1,000 referral bonus. Due to our rapid growth, our need for physicians, nurse practitioners and physician assistants, is at an all-time high. CleanSlate has now introduced a program that will award a $3,000 referral bonus to an employee who refers an NP or PA who qualifies. And for a physician candidate who qualifies, we will pay a $5,000 bonus.

To qualify, the referred candidate must be hired and continue to be employed after the 90 day introductory period.

Who can you refer? How about a former co-worker, a friend, or a friend of a friend? Reach out to anyone meeting the job description criteria who you believe would make an excellent addition to our staff.

How do you refer? Go to the CleanSlate Team Member Handbook and click on the Employee Referral Program Submission Form. Fill this out and send it to the VP Talent Acquisition, Scott Snyder, at

The referral will be verified and approved by People Services and the hiring manager. After the referred candidate reaches the end of the 90 day period, and the referring employee is still an active employee of CleanSlate, the bonus is paid.

Who is eligible to receive the bonus? All employees qualify for the referral bonus, except Senior VP and above, People Services department employees and the Hiring Manager or any other individual involved in the hiring decision.

*Enclosed at the end of the Team Member Handbook is the Employee Referral Program Submission Form.
