Chief Executive Officer
As the Chief Executive Officer of CleanSlate, Chris leads the nation’s foremost outpatient medication-assisted addiction treatment provider. With over two decades of executive leadership experience, Chris brings a patient-centered and people-first approach to CleanSlate’s commitment to expanding access to mission-driven care for individuals facing mental and behavioral health substance and alcohol use disorders.
Prior to his role at CleanSlate, Chris served as the President and Chief Operating Officer of D4C Dental Brands, where he supported specialty pediatric and orthodontic providers across 130 locations. He also held the position of Chief Operating Officer at Correct Care Solutions (now Wellpath), a national healthcare provider delivering medical, mental, and behavioral services to nearly 300,000 patients in various settings, including inpatient and residential treatment facilities, civil commitment centers, and local, state, and federal correctional facilities.
Chris began his professional journey after graduating with a bachelor’s degree in engineering management from the United States Military Academy at West Point. He served as an officer in the United States Army and later earned a master’s degree in business organizational management from the University of La Verne in California.
Dr. William Ruby

Dr. William Ruby is CleanSlate’s Chief Medical Officer. A highly accomplished professional with an impressive background in the medical field, Dr. Ruby had dedicated his career to serving patients in areas where they were underserved or neglected within their own communities. His journey led him to work in the correctional medicine field, addressing the healthcare needs of incarcerated individuals in the State of Maryland. Dr. Ruby’s expertise and passion for infectious diseases were further honed during his tenure at Johns Hopkins University School of Medicine, where he contributed to the Division of Infectious Disease. Following his tenure at Johns Hopkins University, he joined Gilead Services as the Director of Medical Affairs.
Dr. Ruby continued his mission-focused work with the Collier County Health Department, offering his medical acumen to benefit the Department of Communicable Diseases and Prevention. Subsequently, he held several leadership roles at Wellpath (Correct Care Solutions), where he played a crucial role in implementing key clinical programs, providing clinical leadership and ensuring quality control for a vast patient population across the nation. Prior to joining CleanSlate, Dr. Ruby served as the Chief Medical Officer for Intermountain Centers for Human Development.
Chief Administrative Officer
As the Chief Administrative Officer, Roger oversees and managing administrative operations at CleanSlate, ensuring seamless organizational processes and optimal resource utilization.
Before joining CleanSlate, Roger held several executive leadership roles, including Executive Vice President of Operations, Senior Vice President of Sleep Lab Operations, Chief Financial Officer, and Vice President of Development and Financial Operations at BioSerenity, Inc. (and its predecessor, SleepMed Incorporated). With over 25 years of experience in the healthcare industry, Roger brings extensive operational and financial expertise in physician practice management, diagnostic testing facilities, and durable medical equipment.
Roger began his professional journey after earning a Bachelor of Science in Accounting from Wake Forest University. He spent nine years in Price Waterhouse’s audit practices in Charlotte, NC, and Johnson City, TN.
Douglas C. McCracken
Chief Operating Officer
As the Chief Operating Officer at CleanSlate, Doug plays a pivotal role in overseeing the organization’s business operations with a focus on optimizing operational capabilities, enhancing patient satisfaction, driving same-store growth, and spearheading the expansion of CleanSlate’s national footprint.
Before joining CleanSlate, Doug served as the President of the Oncology service line at Alliance HealthCare Services, a leading provider of outpatient Radiology and Oncology services to hospitals, health systems, and physician groups nationwide. His extensive experience at Alliance included leadership roles as the Chief Operating Officer of their Radiology service line and as the Senior Vice President of Finance.
Doug holds a Master of Business Administration from the Kelley School of Business at Indiana University, and he earned his Bachelor of Business Administration from Michigan State University.
Chief Financial Officer
As Chief Financial Officer, Matt drives strategic planning and growth for the organization. He focuses on acquisition and de novo support, payer contracting, and the development of financial, operational, and clinical outcome reporting.
Prior to joining CleanSlate, Matt served as Chief Administrative Officer of Pivotal Healthcare Partners and brings with him over 25 years of senior financial management experience. Notably, he has accumulated 15 years as Chief Financial Officer for healthcare companies backed by private equity. Matt’s expertise extends to mergers and acquisitions, post-acquisition integration, and building robust corporate infrastructure.
Chief Legal Officer and Head of Human Resources
Gina Bonica is the Chief Legal Officer and Head of Human Resources. With deep litigation, regulatory healthcare, and legal generalist background, Gina Bonica is a decisive legal executive and trusted advisor who exercises sound judgment to provide counsel to
organizations in highly dynamic environments that align with the best interests of the company.
Gina Bonica has considerable experience in matters involving fraud and abuse compliance, including self-referral and anti-kickback laws, corporate compliance programs, Medicare and Medicaid reimbursement, self-disclosures and overpayments, health information privacy, licensing, change of ownership, enrollment and corporate and transactional matters involving mergers, acquisitions, due diligence, joint ventures, and other strategic affiliations. She has counseled, in both in-house and outside counsel roles, individual hospitals, health systems, physician practices, and surgery centers on various legal matters.
Prior to joining CleanSlate Centers, Gina served as General Counsel and Chief Risk Officer at Akumin (Nasdaq: AKU), an international healthcare company. Gina oversaw all global legal, compliance, and corporate governance matters on behalf of the company.
Prior to joining Akumin, Gina served as the General Counsel and Chief Compliance Officer to Alliance Healthcare Services, Inc.
Before entering corporate law, Gina was a partner at Kurland Bonica & Associates in New York, where she focused on civil litigation, including federal and state discrimination cases, fraud, commercial litigation, and cases of first impression.
Prior to private practice, Gina served as Assistant District Attorney, Kings County District, New York, in the Major Narcotics Bureau.
Gina received her Juris Doctorate from New England Law in Boston, MA, and her Bachelor’s degree from the University at Albany, SUNY in Albany, New York.
Chief Information Officer
As Chief Information Officer, Dave is responsible for the strategy to mesh CSC’s mission with world-class systems and processes. He works with the Executive Team to drive innovation and judiciously apply novel technologies to both clinical and business challenges.
Dave has over 25 years of senior IT Executive experience. For the 15 years prior to joining CleanSlate, Dave ran an independent advisory practice as a virtual CIO for firms across a variety of industries including healthcare, biotech, and the alternative asset space. Prior to that, he served as the CIO at Kohlberg Kravis Roberts & Co, one of the world’s largest Private Equity firms, and helped it grow from a two-office boutique to a multi-national enterprise.
Dave holds a Bachelor of Arts from Rutgers College, Rutgers University.
Vice President of Behavioral Health
As the Vice President of Behavioral Health for CleanSlate Centers, Scott supports behavioral health staff in identifying and treating the mental and behavioral health needs of patients. He ensures that assessments, counseling services, peer support services, and patient care coordination services adhere to evidence-based practices, applicable accreditation standards, and state statutes.
Scott has 30 years of experience in the behavioral health field and leadership. Prior to joining CleanSlate, he worked five years conducting court evaluations for the State of Kansas’ Sexual Predator Treatment Program. In the years leading up to his work with the sex offender population, Scott worked for Correct Care Solutions as a Regional Behavioral Health Manager, overseeing behavioral health operations at approximately 20 correctional institutions across the Midwest. Further, Scott previously served as the Behavioral Health Coordinator for the Kansas Department of Corrections’ Reception and Diagnostic Unit and the Central Unit at the El Dorado Correctional Facility, as well as a Behavioral Health Professional at various correction facilities in Kansas. Scott received his Bachelor of Science degree in Psychology from Drake University in Des Moines, Iowa, and his Master of Science in Correctional Psychology from Emporia State University in Emporia, Kansas.
Satkiran Singh Grewal, M.D. M. Sc.

Regional Medical Director for Connecticut & Massachusetts
As Regional Medical Director, Dr. Grewal oversees clinical care operations at the CleanSlate Centers in Pennsylvania and Virginia.
Dr. Grewal joined CleanSlate in 2017 as the Center Medical Director in West Springfield, Massachusetts. Before CleanSlate, Dr. Grewal worked at Baystate Children’s Hospital, serving as Division Chief of the Pediatric Hematology-Oncology program. In this role, he acted as Lead Physician for the institution’s patient safety program under the auspices of a multi-center collaborative consortium called “Solutions for Patient Safety.” He’s held associate professor positions at Tufts University School of Medicine and the University of Massachusetts. Dr. Grewal has a Bachelor of Medicine and Bachelor of Surgery from the University of Delhi and completed his residency at the State University of New York at Stony Brook. Additionally, Dr. Grewal holds a Master of Science in Health Care Quality and Patient Safety from the Feinberg School of Medicine at Northwestern University. He is also board-certified in addiction medicine.
Sally Combest, M.D., DABA, FASAM
Regional Medical Director for Arizona, Wisconsin and Kentucky
As Regional Medical Director for Arizona, Kentucky, and Wisconsin, Dr. Combest oversees all patient medical care at CleanSlate Centers located in those states. Her interest in addiction medicine came about from witnessing the devastating impact of drug and alcohol abuse on patients in the operative setting.
Before joining CleanSlate, Dr. Combest’s professional experience included 13 years as a Clinical Associate Professor at the University of Texas Health San Antonio, Senior Medical Adviser at American Business Systems, LLC, and Vice President at Lamar Anesthesia Associates. She is also skilled in physician relations and critical care. Dr. Combest obtained her Doctor of Medicine and completed her anesthesiology residency at the University of Texas HSC in San Antonio. She has a Bachelor of Science in Nursing from the University of Texas Arlington. Since joining CleanSlate in 2018, she has become board-certified in addiction medicine.
Phyllis Losikoff, M.D., MPH

Medical Director of Research and Development
As Medical Director of Research and Development for CleanSlate, Dr. Losikoff oversees the development and applications of clinical outcomes to facilitate the development and implementation of evidence-based innovations targeted at improving patients’ lives. She acts as the Clinical Advisor to the Quality and Regulatory team and a CleanSlate Medical Leadership team member. During her decade with the organization, Dr. Losikoff has also served as the Medical Director for clinics in the South East Region.
Before her role at CleanSlate, Dr. Losikoff was the Assistant Professor at Warren Alpert School of Medicine, Brown University. While at Brown University, her research focused on the Hepatitis C Virus (HCV), elucidating the immunology of early HCV and providing adequate screenings and treatments to high-risk marginalized populations. Dr Losikoff has extensive experience designing and conducting clinical research, resulting in multiple peer-reviewed publications and a U.S. patent. Additionally, Dr. Losikoff has over 20 years of experience providing care to at-risk adolescents and adults suffering from Substance Use Disorder (SUD), with expertise in infectious diseases prevalent among patients with SUDs. She obtained her Medical Degree from Tel Aviv University Medical School and her Master of Public Health from the Hebrew University. She is board-certified in Addiction Medicine and Pediatric Infectious Diseases.
Senior Vice President, Regulatory Compliance
As Vice President of Medical Affairs and Regulatory, Maria leads clinical and operational policy for all CleanSlate Outpatient Addiction Medicine Treatment Centers. She leads and develops plans to achieve national accreditation and state licensure for all CleanSlate centers, ensuring our centers operate at the highest standards. She also manages the organization’s research, review, audits, and analysis. Her work focuses on creating impactful improvements for CleanSlate employees and patients. Throughout her career in medical affairs, regulatory, and compliance, Maria has achieved excellence in organizational planning and corporate compliance, contributing to enhanced service delivery to patients suffering from substance use disorder.
Before joining CleanSlate in 2017, Maria’s professional experience included over two decades with Mercy Medical Center in Springfield, Massachusetts, where she worked her way up from a registered nurse to Chief Quality Officer. Over her career at Mercy Medical Center, she oversaw patient satisfaction, patient safety, and regulatory compliance. Additionally, Maria received her Bachelor of Science in Nursing from the College of Our Lady of the Elma. She is a Licensed Registered Nurse in Massachusetts and a Certified Professional in Healthcare Quality.
Tonya Bales, CPA
Senior Vice President of Revenue Cycle Management
As Senior Vice President of Revenue Cycle Management, Tonya oversees central billing operations, coding/documentation, and revenue recognition responsibilities for CleanSlate. Since joining CleanSlate in 2018, Tonya has led the team to implement new systems, develop internal departments, and improve processes to ensure a more efficient and effective experience for employees, providers, and patients.
Before CleanSlate, Tonya held management positions at leading healthcare organizations. She served as Director of Managed Care and Contracting with Healogics Specialty Physicians, the Nation’s largest provider of advanced wound care services, and Affiliate Chief Financial Officer of TeamHealth, one of the largest providers of hospital-based clinical outsourcing. She also served as the President and Owner of Bales CPA & Consulting, handling accounting functions, healthcare consulting, and individual business tax planning. In addition to being a Certified Public Accountant, Tonya holds a Master of Accounting and a Bachelor of Science in Accounting, both from the University of Tennessee, Knoxville.

Director of Clinical Precepting and EMR Training, Area Clinical Director
As an Area Clinical Director in Massachusetts, Nicole oversees several offices and provides medical leadership along with interdepartmental collaboration as well as direct patient care. Nicole also functions as the Director of Clinical Precepting and provides training, education, and leadership for CleanSlate’s Precepting Team to ensure onboarding medical providers are adequately prepared for their roles.
Before joining CleanSlate 8 years ago, Nicole taught Clinical Laboratory Medicine for Northeastern University’s Physician Assistant Program in Boston and held Physician Assistant positions at the Massachusetts Department of Mental Health, Asthma and Allergy Physicians, Northwest Medical Oncology Specialties in Washington State, and in infectious disease with a private practice in Florida. She is also a certified Medical Technologist with extensive laboratory experience. Her professional distinctions include fellowships with the American Academy of Physician Assistants and Massachusetts Association of Physician Assistants, associate member of the American Society of Addiction Medicine, and certifications with the National Commission on Certification of Physician Assistants and the American Society of Clinical Pathologists. She has a Bachelor of Science in Medical Laboratory Science and a Master of Science in Physician Assistant Studies from Northeastern University..

LGBTQ+ adults are almost twice as likely to experience a substance use disorder, according to research from SAMHSA. Additionally, members of this community often face stigma and discrimination that can impact substance use. At CleanSlate, we do not tolerate discrimination in any form. We offer specialized treatment options for our LGBTQ+ patients including support groups that address the unique concerns and needs of gay, lesbian, bisexual, transgender and queer patients. We believe the best addiction treatment requires treating the whole person. Our integrated care model combines medication-assisted treatment, primary care services and behavioral health therapies to address co-occurring behavioral health issues which can contribute to addiction, such as depression and anxiety.
We understand racial disparities exist in many forms throughout our society, and at CleanSlate we place extra effort on ensuring all patients have access to the highest quality care. Many veterans suffering from addiction experience co-occurring behavioral health issues, such as depression or post-traumatic stress disorder, stemming from their military experience. Our care providers receive specialized training through PsychArmor, which prepares them to better care for our military and veteran patients. Women often face different challenges when it comes to their addiction and recovery. Research shows factors such as physical or sexual trauma, financial independence, pregnancy and childcare can influence the recovery process for women. At CleanSlate, our staff understands substances affect women differently than men, so we develop treatment plans specifically oriented to address these challenges.
